What does a senior care manager do?
A senior care manager, sometimes called a geriatric care manager, helps older adults and their families navigate the many decisions involved in aging, care, housing, and daily support. At CareZen Concierge, we assess each client’s needs, coordinate care services, communicate with families, organize important health information, assist with appointments, and help create a plan that supports safety, comfort, and quality of life.
Our role is to simplify the process for families, especially when care needs become overwhelming, confusing, or time-sensitive.
When Is It Time to Call a Senior Care Manager?
You don't have to wait for a crisis before seeking guidance. Many families reach out when caring for an aging loved one begins to feel overwhelming or uncertain. A Senior Care Manager helps bring clarity, coordination, and peace of mind before small challenges become larger ones.

Your parent has recently been hospitalized or is preparing for discharge.
You're concerned about memory loss or changes in daily functioning.
Managing appointments, medications, and healthcare has become overwhelming.

How is CareZen Concierge different from a social worker?
A social worker often works within a hospital, medical office, government agency, or community program and may be limited by the scope of that organization. CareZen Concierge provides private, personalized senior care management directly for families.
We are able to spend more time understanding the client’s lifestyle, preferences, home environment, care needs, family concerns, and long-term goals. Our support is more hands-on, concierge-level, and relationship-based, helping families coordinate the many moving parts of senior care.
How is CareZen different from a home care agency?
A home care agency usually provides caregivers who assist with daily tasks such as bathing, dressing, meal preparation, companionship, and light housekeeping.
CareZen Concierge is different because we do not simply provide caregivers. As a senior care management and geriatric care management service, we help families evaluate the bigger picture. We can assess care needs, recommend appropriate resources, coordinate with care providers, assist with senior housing options, communicate with family members, and help oversee the care plan.
In many cases, we work alongside home care agencies to make sure the client receives the right level of support.
Is senior housing placement free?
Yes. Our senior housing placement service is typically free for families. When a family chooses a senior living community through our referral, the community may compensate CareZen Concierge directly.
There is no cost to the family for this placement service. Our goal is to help families compare appropriate options such as assisted living, memory care, residential care homes, independent living, or other senior living communities based on care needs, budget, location, and personal preferences.
Do you help with assisted living and memory care?
Yes. CareZen Concierge helps families explore assisted living, memory care, residential care homes, independent living, skilled nursing options, and other senior housing solutions.
We take time to understand the client’s care needs, personality, routines, safety concerns, budget, location preferences, and family priorities. From there, we help identify communities that may be a good fit and guide families through the selection process.
Do you work with families outside Orange County?
Yes. CareZen Concierge primarily serves families in Orange County and surrounding Southern California areas, but we may also assist families outside the area depending on the situation.
For families who live out of town but have a loved one in Orange County, we can serve as a local point of contact, helping monitor needs, coordinate services, attend appointments when appropriate, and keep family members informed.
Can you help if my parent is in the hospital?
Yes. If your parent is in the hospital, CareZen Concierge can help your family understand next steps and plan for a safe transition after discharge.
We can assist with care coordination, post-hospital planning, communication with providers, reviewing support needs, and exploring options such as home care, rehabilitation, assisted living, memory care, or other appropriate services. Hospital discharge can happen quickly, and we help families feel more prepared during a stressful time.
Do you manage medications?
CareZen Concierge can help organize medication information, track medication lists, provide reminders, and communicate concerns to the family or appropriate healthcare providers.
However, we do not prescribe medications or make medical decisions. Medication changes, dosing questions, and clinical concerns should always be handled by the client’s physician, pharmacist, or licensed healthcare provider.
Do you visit seniors in person?
Yes. Depending on the selected service plan and the client’s location, CareZen Concierge can provide in-person visits to check on the client’s well-being, review care needs, observe the living environment, assist with appointments, and provide updates to family members.
In-person visits are especially helpful for families who live far away, have busy schedules, or want another trusted set of eyes on their loved one’s care and daily situation.
