CareZen Concierge was created to bring calm, clarity, and compassionate guidance to families navigating senior care decisions. Our story began with a shared belief that aging adults deserve thoughtful support, trusted advocacy, and a clear path forward through every stage of care.

Today, CareZen Concierge provides senior care management, care coordination, senior housing placement, and real estate transition support for families throughout Orange County.

A Heart for Senior Care Management

Our co-founder Jamie Pascasio-Tran has worked in the senior care industry for more than 10 years. Her experience gave her a firsthand understanding of the challenges families face when trying to coordinate care, communicate with providers, and make important decisions for an aging loved one.

Jamie believed senior care needed more than daily support. It needed heart, structure, compassion, and a more thoughtful way to guide families through each stage of aging.

A meaningful trip to Japan in the spring of 2015 helped inspire the vision for CareZen. The simplicity, calm, and intentional beauty she experienced there became part of the foundation for the company’s approach. Today, Jamie brings that same sense of care and calm to CareZen Concierge’s senior care management services, helping families feel more supported, informed, and at peace.

Real Estate Experience Meets Senior Living Guidance

Our co-founder Chan M. Tran brings more than 25 years of real estate experience to CareZen Concierge. Throughout his career, Chan has worked with families through many of life’s major transitions, giving him a deep understanding of how personal, emotional, and complex housing decisions can be.

His connection to senior care began early in his real estate career through his involvement in opening live-in board and care facilities. That experience helped shape his understanding of senior living, residential care homes, and the importance of finding the right environment for each person’s needs.

At CareZen Concierge, Chan’s background supports families who are exploring senior housing placement, downsizing, real estate transitions, assisted living, memory care, or residential care options. His role helps bridge the gap between care planning and housing decisions, giving families a more complete and supportive experience.
